Surveys are complicated. Our Rabbi says: don't ask a question you if you don't want to hear the answer. If you do a survey, be clear about what information you are looking for. We did a Surveymonkey survey a few years ago, wanting it to be easy, online and easily accessible without necessitating too much time. As with most surveys, we had about a 30% participation rate (but we had 400 children in our school, so that gave us a pretty good sense of where we were). With such a small school, I would recommend that if you CAN call, you should. Better yet, create small focus groups so people can share ideas together. When we did that, the responses dialog was great and even better; some of the participants became leaders in the school.

I see that you posted a while ago. I am new to the network and just saw your request. I have a survey I used with my religious school parents that worked quite well. I used Survey Monkey. If you are still looking for a tool, let me know and I will share with you.
